To provide outreach, supportive case management services, and advocacy for underserved populations in Northern California. 

Outreach to Outcomes is an indigenous and minority led organization and has been created to help fill a need in our community. Development of a culturally sensitive team focused on outreach and case management services, specifically designed to increase access and availability of resources to underserved and vulnerable populations in primarily Butte and Glenn counties.
